Create a digital work order using the Form Builder so field staff can capture all required data directly from the mobile app. This method is an **alternative approach to scheduling jobs** using the Work module and allows your field staff to select a Form in the app and complete it before it syncs back.

Use this approach if your team is not using the Calendar or Scheduling features in Dusk FSM. Instead of assigning jobs, field staff can open the mobile app, select a Form, capture the required data, and submit it directly. This workflow is ideal for ad hoc inspections, audits, or data capture tasks where no planned schedule or dispatch is required.

### ⚙️ Prerequisites
- Admin or Coordinator role with access to **Operations → Forms**
- Dusk FSM mobile app installed for field users (iOS or Android)

### 🧭 Web Path
`Operations → Forms → Form Builder`

### 🚀 Steps to Create
1. Go to **Operations → Forms → Form Builder** and select **Create New**.  
2. Name the form, e.g., *Asphalt Field Work Order*.  
3. Choose one of the following:  
   - 🧱 **Drag & Drop** fields manually  
   - 🤖 **AI Generate:** describe your form using a detailed prompt (e.g., industry, equipment hours, materials used, role)  
   - 📄 **Upload PDF:** attach a paper form; ChatGPT will recreate the layout  
4. Edit fields, reorder, or delete as needed.  
5. On the **Settings** cog on the left sub menu, check that **Show on forms and templates in app** is ticked.  
6. From the **Settings** cog, scroll to **Columns** and select the fields from your new Form that you want displayed for completed records. Choosing key fields such as Name, Reference, or Date makes it easier to quickly identify and review each submission.
7. A shortcut to this form can also be added by ticking **Add to menu** and choosing an icon. This will appear under **Operations->Form Builder->Forms->Your Form**.
8. Click **Save**.

### Steps to View the Form Data
1. Go to **Operations → Forms → Form Builder** and locate the Form you created e.g. *Asphalt Field Work Order*, click on the second icon from the left called **Review Records**.  
2. Here is a list of the submitted Field Forms showing the column headings that were configured when the form was created.
3. View the form, edit the form or generate a PDF from this screen.

Add custom columns in **Settings** to control which fields appear in the submission list.
